Collective Care Companions is honoured to be recognised as an approved DVA community nursing provider, delivering superior home nursing services to veterans and their families in Perth. Our aim is to ensure that eligible individuals receive expert nursing support and medical attention in the safety and comfort of their homes.

All our Services are tailored to the unique needs of each veteran and their family, ensuring compassionate and dignified care that promotes wellness and independence.

Eligibility

If you are a veteran or a war widow/widower holding a DVA Gold or White Card, you may be eligible for government-funded community nursing services. You must also undergo an assessment to determine your needs for clinical and/or personal care.

You must also obtain an authorised referral from:

A General Practitioner (GP)

A Nurse Practitioner specialising in community nursing

A Treating Doctor in a Hospital

A Hospital Discharge Planner

After receiving a referral, our Senior Nurse will guide you through the next steps.

Fees and Charges

If you qualify for assistance from the DVA Community Nursing Program, there’s no cost to you at all.
